office and equipment inspection form.

receipt of application payment
(iii) Valid
business license
(iv)Tax
Payer Identification Number (TIN)
(v)
Certificate of tax clearance
(vi) Certificate
of registration/incorporation from BRELA
(vii)
Identity Card (Nation ID/Driver's License/Voter ID)
(viii)
Working Permit for non-citizen of Tanzania
